Eye Area Aesthetics
Instead of saying
Blepharoplasty or eyelid surgery, I prefer the term eye area aesthetics.
Excess skin, fat, bags, lines, and wrinkles around the eyes can make a
person look older and more tired than they are. My approach is to
maintain the natural proportion and bring out what is natural. That is, to
regain the gaze in the eyes. But changing that gaze is not my style.
That’s why I don’t prefer applications like “cat eye,” “fox eye,” and
“almond eye” that are done at a young age.
My Approach
The most important thing in my approach is this: Why did this excess
occur? What is the problem?
We need to determine this first. Here, of course, I am talking about
physical reasons.
Also, this surgery is an emotional surgery for me. I think that each of the
lines around the eyes has a meaning. In my practice, I have encountered
many patients who lost their fathers at a young age. As a father of a
daughter, this situation affects me particularly. Therefore, this surgery is
not only a technical process for me, but also an emotional one.
Why Does it Occur?
Changes around the eyes are generally associated with:
Aging (especially after 50-60 years of age)
Genetic structure
Racial characteristics
For example:
In Caucasian people, sagging may be more pronounced due to thin skin.
In African people, sagging may appear later due to thick skin.
In Mediterranean people, sagging may occur earlier due to sun exposure
and facial expressions.
Surgical Planning
In this surgery, we mark and remove the excess tissues.
However, the most important point here is balance. Excessive removal
can lead to an artificial and exaggerated appearance.
For example: If the lower eyelid is in moderately good condition but too
much skin is removed from the upper eyelid,
the balance is disrupted. The upper and lower eyelids need to
complement each other.
Which Tissues Are Treated?
If the excess is only skin, only the skin is treated.
If there is excess fat tissue, these tissues are adjusted or removed.
One of the most important points is this: If there is a problem in the
muscle structure that causes the fat tissue to herniate, this muscle tissue
also needs to be repaired.
Eyebrow and Forehead Effect
Excess hair around the eyes may not only be caused by the eyelids.
Often:
Downward pressure of the eyebrow
The effect of the scalp
contributes to this appearance. In this case, the eyebrow needs to be
lifted or fixed in the appropriate position.
Eyebrow Lifting Techniques
There are two main methods for this procedure:
1. Lifting from within the scalp: In this method, the incision is hidden in
the scalp,
and the eyebrow is lifted upwards. However, it may droop slightly over
time.
Therefore, it is necessary to lift more during the surgery. This can lead to
an exaggerated appearance in the early stages.
2. Removal from above the eyebrow: In this method, skin is removed just
above the eyebrow. It is a more controlled and predictable method.
The biggest concern here is scarring. However, when the incision is
placed appropriately at the eyebrow line,
and carefully closed, the scar is usually at an acceptable level.
My observation is that with the right technique, this scar does not pose a
serious problem.
Conclusion
The aim in periorbital aesthetics should be:
To reduce excess skin
To maintain a natural expression
To eliminate a tired look.
The main goal is not to change the gaze, but to restore it.
